Ouch, that was definitely an old plan, the stuff from the 80s and 90s is like an old car that insurance needs to be reviewed and updated. I would tell the claims that you have an ongoing disability, as they pay in full to the dates the doctor writes and if you go in there and get the doctor note the same day the payout is small.

If it's disability though like other companies it pays through from the day of disability to the the day of the doctors report. Then you need to revisit periodically with them every month, your coverage is in force to get another payout as long as you remain disabled.

Combined Insurance in Calgary, Alberta - Great experience on claims

Printed my claim form. Had to explain what total disability means to this company if you can not perform your normal duties then you are unable to work period. So that was good. Paid on my healing time at home for 2 months plus my ICU. I had a quad accident which went bad fast.
